Identification and Evaluation of Nursing Errors in Kowsar Hospital by SHERPA

Background: Medical errors, including human are one of the most important and sensitive issues in health system. Human errors can endanger patients’ life during service provision. Therefore, healthcare workers should ensure safety by reducing errors. This study aimed to identify evaluate among nurses working at an Iranian Hospital. Methods: is a cross-sectional which was carried out using SHERPA method 2018. After explaining purpose study, data were collected observation interview. Using hierarchical analysis method, tasks subtasks determined, finally, worksheet on completed analyzed. Results Discussion: Thirty-two covering 252 related nursing profession investigated, detected. Identified included action, revision, retrieval, communication, selection responsible for 60, 23, 3, 12, 2 percent respectively. In terms level error risk, 16 unacceptable, 96 undesirable, 107 acceptable, need 33 acceptable with no revision. Conclusion: Nurses' more action because nature their work, so hospital authorities prevention prioritize these
